UNCONTROLLED TEMPERATURE IN AN OIL TANK

EVENT REPORTED by SAM GUARD®

Detected an uncontrolled increase in temperature in an oil tank related to the ethylene compressor. As a result the heating in the oil tank immediately stopped working.

MAINTENANCE TEAM FINDINGS

Due to the SAM GUARD® alert the plant team found a worn mechanical seal of the ethylene compressor in stage 2, therefore, more hot oil goes through it than it should. As a result, the heater stopped working.

COST / DOWNTIME AVOIDED

A worn seal can lead to a sudden stop of the ethylene compressor and shutdown of the entire facility.

Following SAM GUARD® alert Pressures were frequently monitored, and it was decided to change the seal as soon as possible at the next planned shutdown.
